Aboriginal and Torres Strait Islander peoples advance care planning, palliative care and end of life information bundles

The Healthily GoShare Plus platform is now offering Aboriginal and Torres Strait Islander Peoples Advance Care Planning, Palliative Care, and End of Life information bundles.
This initiative is endorsed by esteemed South Australian Aboriginal community leaders, including Uncle Major ‘Moogy’ Sumner, Aunty Michelle Jacquelin-Furr, Uncle Adrian Ware, and Margi Sumner, who share insightful video messages on these critical topics.

The aim is to facilitate open and meaningful conversations about end-of-life planning within the Indigenous community, drawing on cultural wisdom and traditions.

GoShare Plus, a user-friendly online tool, delivers educational content via SMS to patients. The Country SA PHN is generously funding GoShare until June 30, 2024, to support general practices in enhancing quality improvement and preventative health activities in country South Australia.

Engaging in discussions about end-of-life matters is crucial for understanding, acceptance, and preparedness. While these conversations may be challenging, they are essential to ensuring that individuals’ wishes are respected and that loved ones receive necessary support during difficult times.

To access the Aboriginal and Torres Strait Islander Peoples Advance Care Planning bundles, practitioners can complete the Expression of Interest form. For inquiries or assistance, the support team at support@countrysaphn.com.au is available to provide resources and support for facilitating these important conversations within the community.
